How to Apply For A PhilHealth ID


PhilHealth, as we know it is one of the Philippine organizations to aid employees in their health insurance. However, for them to verify that you are a member of the organization, you would need legitimate identification. One is by securing your PhilHealth number, another is by having an ID. Yeah we have the classic laminated ID for PhilHealth; it is the one where you need a 2×2 photo. Now, PhilHealth is offering its members a digitized, PhilHealth ID card.


This new ID does not only serve as a valid ID. Of course, it proves your membership with the organization and at the same time, it makes you eligible to avail its benefit services; one if which is the life insurance. But first, you of course need to know how to get a PhilHealth ID.

So how do you get a PhilHealth ID card?

If you are currently employed, then you’re good because your company should be the one paying for your contributions. However, if you’re unemployed or self-employed, you need to register yourself as an “Individually Paying Member” or a “Voluntary Member.” Furthermore, foreigners currently residing here in the Philippines are also part of this type. You would need to visit the nearest PhilHealth office and do the following:

Step 1: Duly accomplish two (2) copies of the PhilHealth Member Registration Form (PMRF)

Step 2: Submit PMRF to the nearest LHIO together with the following documents:

  • Two (2) 1×1 latest ID picture,
  • Two valid IDs; and
  • Any of the following documents:

For Uniformed personnel of AFP, PNP, BJMP and BFP
